HeartPlan in a box is a quality improvement toolkit for heart failure (HF) management in primary care. Within this cycle you will learn about the burden of HF in Australia, heart failure management in Primary Care and the utilisation of GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) to support practitioners coordinate the care of patients with chronic conditions. This Quality Improvement activity on the management of HF in primary care focuses on defining a process to ensure regular review of patients. Practices should ensure all eligible patients with HF receive a GPMP and/or TCA review. The goal is to improve the management of HF in practice by ensuring patients are notified and undergo regular reviews.
