This QI activity on the optimisation of HF treatment in primary care focuses on ensuring all patients with HF receive the optimal doses of HF medications. The goal is to improve the management of HF in practice by ensuring patients are prescribed the optimal dose and combination of HF medications and are regularly reviewed.
