Heart Plan in a box is a quality improvement toolkit for heart failure management in primary care. Within this cycle you will learn about the burden of HF in Australia, heart failure management in Primary Care and the utilisation of GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) to support practitioners coordinate the care of patients with chronic conditions. This QI activity on the management of HF in primary care focuses on ensuring that all eligible patients with HF have a GPMP and/or TCA and that these have been reviewed in the past 3 months.
